Track health, safety, and environment observations with corrective actions.
Overview
The HSE Observations app records Health, Safety, and Environment
observations across your facility. It captures four types of
observations: unsafe acts (human behaviours that could
lead to incidents), unsafe conditions (physical hazards
in the workplace), near misses (events that almost
caused harm), and positive behaviours (good safety
practices worth recognising). Each observation tracks severity, location,
corrective actions, and evidence photos, creating a comprehensive safety
record.

Reporting an Observation
Any workspace member can report a safety observation:
Navigate to HSE Observations and click
New Observation.
The system auto-generates a unique number (HSE-0001, HSE-0002,
etc.).
Select the Observation Type: unsafe act, unsafe
condition, near miss, or positive behaviour.
Set the Severity: low, medium, high, or
critical.
Choose the Location — select the building
and section where the observation was made.
