Track unsafe acts and conditions for proactive hazard identification
Overview
Safety Behaviour Observations (SBO) are the cornerstone of proactive safety management.
Instead of waiting for incidents to happen, observers patrol the site and document
unsafe acts (people doing something dangerous) and unsafe conditions
(hazardous situations in the environment). High observation volumes with strong closure
rates are a leading indicator of good safety culture.
Configuring Unsafe Items
Before recording observations, define the hazard classification library for each project.
Each item is categorized as either an Unsafe Act or an Unsafe Condition.
Go to Safety Observation > Unsafe Items and click New Item.
Select the Type:
Act — e.g., "Not wearing PPE", "Improper lifting technique", "Bypassing safety guard"
Condition — e.g., "Unprotected excavation edge", "Blocked emergency exit", "Missing barricade"
Enter a clear, specific Name for the item.
Click Save.
Tip: Pre-populate the unsafe items list at project kickoff. Reference your
organization's HSE manual for a standard set of acts and conditions.
