Report safety incidents from near misses to fatalities
Overview
When something goes wrong on site — from a close call to a serious injury — it must be
formally documented. The Incident Register captures every safety incident with full
details: what happened, who was involved, immediate actions taken, and the subsequent
investigation with root cause analysis and corrective/preventive actions (CAPA). This
register feeds into safety performance metrics like LTIFR (Lost Time Injury Frequency
Rate) and TRIFR (Total Recordable Injury Frequency Rate).
Reporting an Incident
Incidents should be reported as soon as possible after they occur:
Navigate to Incident Register > Incidents and click Report Incident.
Select the Incident Type from the project's configured types (e.g., "Fall from Height", "Electrical Shock", "Vehicle Collision").
Set the Severity:
Near Miss — no injury occurred, but could have.
First Aid — minor injury treated on site.
Medical Treatment — required professional medical care.
Lost Time Injury — worker unable to return next shift.
Fatality — death resulting from the incident.
Record the Date, Time, and Location.
